ENROLL 

Thank you for your interest in enrolling at Carpe Diem Innovative School – Northwest. As a free, charter school, enrollment is open to students in grades 6 – 12. Students will be enrolled on a first come, first serve basis until the school reaches capacity.

Visit our school to complete the enrollment application. When you come please bring the following items:

  1. Birth Certificate
  2.  Proof of Residence (Mortgage or Rental Statement or Utility Bill)
  3.  Report Card/Transcript
  4. Photo ID of parent (license, ID, or Passport)
  5.  Immunization records
  6. Important Student Records ( ex. Individual Education Plan)
